Party Event Planning Form:
Please Remember: Your Party Event Planning Form is due at least 2 weeks prior to your event. A $50 late fee applies to all forms submitted late. A late form can be (a) a form that is submitted but has incomplete/blank areas not specified or (b) a form submitted within the 2 week period prior to your party.
We ask that you, please:
1. Fill in all the blank fields. If an area of the form does not apply to your event, type "Not Applicable" in the blank field(s).
2. Make sure to indicate the correct address of your facility location and correct name(s) of on-site contact person(s). Please take notice of our "Do Not Play" area of the Party Event Planning Form. If you do not have any specific songs for your "Do Not Play" list, please indicate by typing "Not Applicable".
3. It is your responsibility to provide your DJ with a timeline of events for your event. Your timeline does not need to be exact times (i.e. 6:00pm Sing Happy Birthday), but it does need to be a brief outline of how the event is planned to go. Please email your timeline to your reserved DJ. Contact your booking agent if you do not have your DJ's contact information.
4. Once your form below has been submitted, please email or call your DJ to schedule a conference call or in-person meeting during a day/time that works for you!
Special Note: You can pick out a specific song(s) on your form and change your mind later on. Just inform your DJ about your song change(s) and provide him or her with your new song(s) on a CD. Your DJ is not responsible for downloading songs for you within the 2-week period before your wedding. Why? Most likely, your DJ is working with a handful of event clients at the same time and our guidelines are designed to help your DJ be efficient.
